The Art of the Possible in Digital Quality: Better Measures, Better Incentives, Better Care
Reid Kiser, Bradley Ryan, Rob Reynolds, Matthew Burton · October 2026
Digital quality has made real progress, but most of it has moved paper measures into digital formats. Far less has changed about what gets measured, or how directly measurement supports care. This paper takes stock of where the field is and lays out the next phase: clinically meaningful, computable measures, tied more closely to guidelines, care delivery, and payment. The standards now exist. The remaining work is making clinical goals, reasoning, and exceptions explicit, so one shared definition can drive both the bedside recommendation and the measure.

SMART Guidelines, Smarter AI
Matthew M. Burton, Sivaram Arabandi, Davide Sottara · Independent discussion draft
AI’s biggest contribution to clinical care may come before any clinical decision is made: turning guidelines into logic that runs the same way every time and shows its evidence. That work used to take years and rare expertise. We know; we did a lot of it. This paper, written for guideline publishers, shows how AI can speed it up without giving up that rigor. The bigger payoff is outcomes-based payment: once clinical value computes at scale, every party can run the same measures, see the same evidence, and pay on it.
